This template is intended for clubs, football schools, academies, federations, schools, and other sports organizations using Pulsessivo.ai for youth programs in Italy. The document should be completed by the data controller, adapted to internal safeguarding procedures, and retained in the organization's records. Before operational use, a legal review is recommended in light of Regulation (EU) 2016/679, Legislative Decree 196/2003 as amended by Legislative Decree 101/2018, and any guidance issued by the Italian Data Protection Authority.

4. Purposes for Using Pulsessivo.ai

The organization identified above uses Pulsessivo.ai to support youth-sector operations, including:

  • team administration and access management;
  • training planning, attendance tracking, and athlete availability;
  • monitoring workload, general wellness, and sports performance;
  • secure storage of coaching notes, athlete status updates, and team documentation; and
  • optional integration with compatible wearable devices where enabled by the organization.

Pulsessivo.ai and any smart bracelet are not medical devices and are not intended for diagnosis, treatment, cure, or prevention of disease.

5. Categories of Data That May Be Processed

Depending on how the organization configures the service, the following may be processed:

  • athlete identification data and team information, such as name, category, role, and team assignment;
  • parent or guardian contact details;
  • attendance, training, workload, wellness, performance, and coaching-note data entered by authorized personnel;
  • technical and security data, such as login history, audit logs, device or browser information, and support requests; and
  • where wearable devices are used, special-category data under Article 9 GDPR, including biometric or health-related data such as heart rate, activity levels, and performance metrics.

6. Information Relevant to Use in Italy

  • The organization identified in Section 1 typically acts as the data controller for athlete data entered into the platform.
  • Pulsessivo.ai typically acts as the data processor on behalf of that organization, within the limits of the contract and the instructions received.
  • Access to data should be limited to authorized personnel with a real need to know, in accordance with the principles of minimization, confidentiality, and accountability.
  • Biometric or health-related data should only be processed where there is an appropriate legal basis and, where necessary, explicit consent under Article 9 GDPR.
  • For information-society services offered directly to minors in Italy, Article 2-quinquies of the Italian Privacy Code sets 14 years of age as the threshold for autonomous consent; for organization-managed youth sports programs, a parental-authorization workflow and controlled account activation are nevertheless recommended.

7. Parent / Guardian Declarations and Consent

  • I understand that I may exercise the rights provided by Articles 15 and following of the GDPR, subject to applicable law, by contacting the organization identified in Section 1 first.

Optional biometric / wearable consent

Complete this subsection only if the organization uses smart bracelets or other compatible wearable devices.

  • I give my explicit consent to the collection and processing of the athlete's biometric or health-related data, including heart rate, activity levels, and performance metrics, through Pulsessivo.ai and compatible wearable devices exclusively for general wellness, sports monitoring, and training-support purposes.
  • I understand that this consent may be withdrawn at any time, without affecting the lawfulness of processing carried out before withdrawal.

8. Minor Athlete Acknowledgement (Recommended)

Where appropriate in light of the athlete's age and maturity, the organization should explain the service in language the athlete can understand and collect an age-appropriate acknowledgement or assent.

10. Retention and Contacts
